2️⃣ Comprehensive Assessment – Our clinician evaluates how your condition impacts your ability to perform job duties.
3️⃣ Documentation & Certification – If you qualify, we complete the necessary FMLA paperwork for your employer.

FMLA Evaluation Fees

Intermittent FMLA Leave

💼 Intermittent FMLA Leave: $450


This applies to situations where you need to take leave in smaller, recurring periods—such as for ongoing therapy, flare-ups of a mental health condition, or medical appointments.


⏳ Appointment Duration: 60 minutes

Continuous FMLA Leave

💼 Continuous FMLA Leave: $600

This applies if you need to take leave for three or more consecutive days due to a serious mental health condition that prevents you from working.

Important Information About FMLA & Paid Leave

✅ Federal FMLA provides job protection—but not paid leave. Some employers may offer pay by allowing you to use sick time, vacation days, or other benefits.
