Supplemental Workplace Functional Assessment Report™



If you already have a diagnosed medical or mental health condition and need clear, structured documentation for a workplace accommodation request, this service provides a focused functional assessment designed to translate clinical information into job-relevant language employers are required to evaluate.

Many accommodation requests are delayed or denied because medical documentation does not clearly explain how a condition impacts essential job duties. This assessment helps bridge that gap by organizing existing medical information into a concise, workplace-focused report.

This service is appropriate for individuals who already have documentation from a physician, therapist, or other qualified provider but need assistance clarifying the functional impact of their condition within a workplace context.

Low Complexity - $450

(For individuals with up to two (2) existing diagnoses)

This fee applies if you already have a diagnosed mental health condition or established disability with supporting documentation. It includes:

✅ 45-Minute Structured Interview: A focused consultation to understand your job duties, workplace environment, and how your documented condition impacts your ability to perform essential functions.


✅ Review of Existing Medical Documentation: Review of relevant records or provider documentation you already have available.


✅ Functional Limitations Analysis: Identification of how documented symptoms may affect specific workplace tasks, environments, or schedules.


✅ Written Report: A report summarizing functional impacts, workplace considerations, and regulatory guidance based on the information reviewed.


✅ Completion of Workplace Accommodation Forms: Completion of employer-provided accommodation documentation when applicable.


Available to all U.S. employees, nationwide.

Moderate Complexity - $650

(For individuals with up to four (4) existing diagnoses)


This fee applies if you already have a diagnosed mental health condition or established disability with supporting documentation. It includes:

✅ 75-Minute Structured Interview: A focused consultation to understand your job duties, workplace environment, and how your documented condition impacts your ability to perform essential functions.


✅ Review of Existing Medical Documentation: Review of relevant records or provider documentation you already have available.


✅ Functional Limitations Analysis: Identification of how documented symptoms may affect specific workplace tasks, environments, or schedules.


✅ Written Report: A report summarizing functional impacts, workplace considerations, and regulatory guidance based on the information reviewed.


✅ Completion of Workplace Accommodation Forms: Completion of employer-provided accommodation documentation when applicable.

Reasonable Accommodation 90-Day Packages

Initial RA Certification / Reconsideration / Appeal - $900


(This service is designed for individuals who already have a diagnosis from a licensed provider in their State. No more than five (5) diagnoses.)

This fee applies if you already have a diagnosed mental health condition or established disability with supporting documentation. It includes:

✅ Comprehensive Evaluation & Consultation

✅ Review of Existing Medical Documentation: Review of relevant records or provider documentation you already have available.


✅ Functional Limitations Analysis: Identification of how documented symptoms may affect specific workplace tasks, environments, or schedules.


✅ Written Report: A report summarizing functional impacts, workplace considerations, and regulatory guidance based on the information reviewed.

✅ Coordination with treating provider for completion of workplace accommodation forms or a custom letter (when clinically recommended) to be signed by your treating provider.


Includes: Three (3) follow-up sessions (45 minutes each) and up to two (2) hours of email/phone support for 90 days after receiving your accommodation documentation.


Available to all U.S. employees, nationwide.

Comprehensive Diagnostic RA Evaluation & Certification - $1500

(Available in Maryland & DC ONLY)

If you have not been previously diagnosed but suspect you may have a condition such as Major Depressive Disorder, PTSD, or an anxiety disorder, a more in-depth evaluation is required. This includes:

✅ Comprehensive Evaluation & Consultation


✅ In-depth review of records and relevant medical history for medical diagnoses to be included in your request

✅ Psychological assessment & diagnostic testing (for conditions such as PTSD, Major Depressive Disorder, Anxiety, etc.)

✅ Scoring & interpretation of results

✅ Functional Limitations Analysis: Identification of how documented symptoms may affect specific workplace tasks, environments, or schedules.


✅ Written Report: A report summarizing functional impacts, workplace considerations, and regulatory guidance based on the information reviewed.

✅ Coordination with treating medical provider, when clinically recommended, to be co-signed by your treating provider (as applicable).

✅ Completion of workplace accommodation forms (if required). 


Includes: Three (3) follow-up sessions (45 minutes each) and up to two (2) hours of email/phone support for 90 days after receiving your accommodation documentation.
